We are beginning to plan for our annual Thanksgiving Dinner box distribution. If you’d like to receive items for a Thanksgiving meal, please complete this form of item preferences and dietary restrictions*.

All forms must be submitted no later than October 31st. If you require assistance and/or access to a computer to complete the form, you can contact Brandi in the office.

Boxes must be picked up on Thursday, November 20, or Friday, November 21, at Teller.

*Please note that we will do our best to match your requests, however we cannot guarantee you will receive everything you've requested. Dietary restrictions will take precedence over item preferences.
